5/15/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Jashim upon arrival to the home setting. arrival to the home setting, Jashim appeared to be in a positive mood due him engaging in general conversation with QMHP-C. Mother informed QMHP-C on how Jahsim continues to get in trouble in the school setting and runs away because he is unable to have his way. 2. Behavior: Jashim presented cooperative and normal behaviors as he did not need any redirection from QMHP-C as well as him answering open -ended questions and being praised by QMHP-C. At times within the session, be became defensive with his mother as well as grandmother as they were addressing his behaviors. 3. Behavior: During this portion of the session, QMHP-C Met with Jahsim and mother at his residence for family session focusing on negative behavior patterns, objective, and treatment plan goal, setting boundaries, anger management and control issues. 1.Intervention: QMHP-C explored with Jashim the trigger to his current emotional state and encouraged Jashim to utilize his positive coping skills to help regulate his mood. QMHP-C engaged Jashim through the use of active listening, asking open-ended questions and providing empathetic responses. 2. Intervention: QMHP-C encouraged Jashim to recognize that his coping skills should not be fixed and that they can change depending on the mood and situation. QMHP-C ended session by reviewing interventions with Jashim and encouraging his to provide feedback on the session. 3. Intervention: QMHP-C discussed the meaning of control and how to deal with not having the ability to control his anger and behaviors. QMHP-C encouraged the mother to have family discussions to involve Jahsim grandmother so they both can be on the same accord when disciplining Jahsim. QMHP-C discussed with Jahsim and the mother short- and long-term goals as well as boundary setting techniques. 1. Response: Jashim appeared to be receptive to QMHP-Cs’ interventions and services that were provided to him during the session. Jashim stated that although his positive coping skills help him to relax and remain positive when he is moderately upset or sad, they do not seem to help him when he experiences overwhelming feelings of anger or sadness. 2. Response: Jashim stated that he is still exploring what coping skills work for him. At the end of the session, Jashim indicated to QMHP-C that he is going to continue to do what works for him (with the assistance of the QMHP-C). 3. Response: The mother also reported that Jahsim always must have the last word and he continues to defy her wishes. Jahsim reported that he listens to what his mother has to say but he always yells at her for nothing. The mother expressed her frustration with Jahsim and reported that she does not know what else needed to be done about his negative behavior.
